  • Come in 5 minutes earlier if you have the forms filled out, if not, come in 15 minutes earlier.
  • Bring any Insurance cards and a photo ID
  • Bring Current glasses or contacts you are wearing
    We collect co-pays up front. All fees are due at time of service.
  • Be prepared to have a thorough comprehensive exam catered to your unique needs!
